Advance Directive Registry—Arizona

Created in 2004 by the Arizona State Legislature, the Arizona Advance Directive Registry is a secure online repository where Arizona residents can store their advance directives, including Living Wills, Medical Powers of Attorney, Mental Health Powers of Attorney, or a combination of these. Advance directives that have been filed with the Arizona Advance Directive Registry can be accessed online by patients, loved ones, and health care providers from anywhere in the world.
